King of the Monsters 2 no Sega Genesis é um jogo ligeiramente diferente da versão original do Neo Geo. Enquanto o original permite que você jogue com um dos três monstros diferentes para passar fases e lutar contra monstros (não-jogáveis) como inimigos, a versão Genesis foi re-trabalhada em um lutador um-contra-um semelhante ao primeiro jogo de King of the Monsters. Além de Farmardy, todos os personagens da versão Neo Geo são jogáveis. Para ganhar, o jogador deve derrotar todos os outros monstros e um clone do monstro jogado em uma luta. Os jogos acontecem em áreas construídas a partir dos níveis originais, e apresentam paisagens destrutíveis e inimigos secundários, os quais podem ser esmagados para ganhar pontos e power-ups.
